Service Description
**CURRENTLY OFFERED TO ILLINOIS RESIDENTS ONLY** This virtual consultation is designed determine your eligibility for GLP-1 medication for weight loss. During your appointment, your medical history and weight loss goals will be reviewed to determine if treatment is appropriate. Please note that scheduling a consultation does not guarantee a prescription, as eligibility is based on a medical evaluation and clinical judgement.
Cancellation Policy
What if I'm Late: Please log in a few minutes before your scheduled appointment time. If your more than 10 minutes late, your appointment will be considered a missed appointment and will need to be rescheduled. The $100 virtual consultation fee is non-refundable, and a new consultation fee is required to schedule another appointment. What if I need to reschedule my appointment Reschedule with at least 24 hours' notice is no penalty. The consultation fee transfers to the new appointment. If less than 24 hours, the consultation fee is forfeited and a new consultation fee is required to book another appointment What if I need to cancel my appointment Cancel with at least 24 hours' notice, consultation fee is non-refundable but remains as a credit for a future appointment (or, if you prefer, is forfeited-your choice) Less than 24 hours, consultation fee is forfeited, and a new consultation fee is required to book another appointment.
