Service Description
Embark on a personalized weight loss journey with the guidance of our experienced nurse practitioner at LaChic Aesthetics. Based on your medical history and current health goals, if you meet the criteria for GLP injection medications this will work well for you to achieve your goal. Book your appointment to start your transformation! Eligibility: 1) Have you ever been diagnosed with any of the following? If so, you are not eligible for this service. Diabetes | Pancreatitis | Gastroparesis | Prediabetes 2) If you, your siblings, or your parents have a history of the following conditions, you are not eligible. Medullary thyroid cancer | Multiple endocrine neoplasia type 2 3)If you are currently taking any of the medications below, you are not eligible. Insulin | Sulfonylureas | Opioid pain medication 4) Your current BMI must be above 30. Or you are not eligible for this service. 5)You must be an Illinois resident. Or you are not eligible for this service.
Cancellation Policy
As a courtesy to our providers please provide a minimum of 24 hours notice should you need to cancel or reschedule an appointment. If a patient cancels or reschedules less than 24 hours prior to their appointment, they will be charged $100. If a patient fails to show up for their appointment without prior notice they will be charged $200. LaChic Aesthetics understands that there are circumstances that may warrant a waiver of this policy. LaChic Aesthetics reserves the right to waive its Cancellation Policy at its sole discretion, on a case by case basis. Generally, a waiver of the cancellation fee will only be granted in the case of emergencies such as inclement weather.
