Eligibility Evaluation for GLP-1 plan
Service Description
This evaluation session is an essential step to determine your eligibility for our weight loss program. Due to specific health criteria and restrictions, not everyone may qualify for this treatment. During the session, a specialist will carefully assess your medical history, current health status, and personal goals to ensure this program is a safe and suitable option for you. Eligibility: 1) Have you ever been diagnosed with any of the following? If so, you are not eligible for this service. Diabetes | Pancreatitis | Gastroparesis | Prediabetes 2) If you, your siblings, or your parents have a history of the following conditions, you are not eligible. Medullary thyroid cancer | Multiple endocrine neoplasia type 2 3)If you are currently taking any of the medications below, you are not eligible. Insulin | Sulfonylureas | Opioid pain medication 4) Your current BMI must be above 30. Or you are not eligible for this service. 5)You must be an Illinois resident. Or you are not eligible for this service.
Cancellation Policy
As a courtesy to our providers please provide a minimum of 24 hours notice should you need to cancel or reschedule an appointment. If a patient canceled or rescheduled with less than 24 hours' notice but more than 1 hour in advance will occur a $100 fee. No-shows, cancellations/reschedules made less than 1 hour before the appointment or arriving more than 20 minutes late will result in a $200 fee. Arriving late does not guarantee you will be seen. Service will be at the discretion of the nurse practitioner. LaChic Aesthetics understands that there are circumstances that may warrant a waiver of this policy. LaChic Aesthetics reserves the right to waive its Cancellation Policy at its sole discretion, on a case by case basis. Generally, a waiver of the cancellation fee will only be granted in the case of emergencies such as inclement weather.
